To request a transcript, simply complete the required form by clicking the button below.

Important Note for DMACC Credits: If you earned any DMACC credits while attending NPHS, please request a transcript directly from DMACC.

No need to fill out this form if you have graduated from North Polk within the last month. For graduates, transcripts are automatically sent out by counselors to colleges in the first week of June for the current year's senior class. Please allow for sufficient receiving and processing time before requesting another transcript.
